Please read the instructions below in entirety before beginning your online application. You should have your credit card ready when you submit your application online to pay the $150 application fee.
You will be asked to create a Login ID and Personal Identification Number (PIN), which will enable you to return to your application as many times as you like before your final submission. Your Login ID and PIN will also allow you to check your application status. Please note that the Login ID is case sensitive.
- Applicants who complete coursework outside the U.S. must have official transcript(s) evaluated by a recognized international credit evaluation service. Official evaluation of all foreign (non-U.S.) coursework must be sent to the Azure College School of Nursing by the service.

English Proficiency
One of the following requirements for English Competency must be demonstrated through one of the following:
- Successful completion of ANY one of the acceptable English competency examinations:
- A minimum score of 500 (200 on computerized version and a 70 on the Internet based version) on the Test of English as a Foreign Language (TOEFL)
- A minimum score of 79% on the Michigan English Language Assessment Battery (MELAB)
- A minimum MELAB converted score of 79% on the Michigan Examination for the Certificate of Proficiency in English (ECPE) Examination
- A minimum score of 725 on Test of English for International Communication (TOEIC)
- A minimum score of 6.5 overall with a 7.0 on the spoken portion of the academic version of International English Language Testing System (IELTS);
- 5- Completion of a Medical program given in English in another country;
- A passing score on one of the USMLE Steps (Step I, Step II CK or Step II CS).
- Completion of a college level course for academic credit in a U.S. institution
Note: Arrange to have two (2) sets of the English test scores sent, one (1) OFFICIAL COPY to Azure College Admissions Office, and one (1) UNOFFICIAL COPY to the School of Nursing.
